GENERAL INFORMATION:

Program Name:          Sun Rose Apartments – Permanent and Transitional Supportive Housing Program

Program Location:      Salinas, CA

Program Schedule:     Monday through Friday 8:30 a.m. to 5:00 p.m. Staff are on-call via cellphone for emergencies. A resident manager lives on the premises for night emergencies.

Program Description: Sun Rose Apartments is both a permanent supportive housing program and a transitional housing program for very low-income individuals with a serious mental health diagnosis, who are all homeless or at-risk of chronic homelessness per the HCD NPLH definitions. The PSH consists of 9 units and the TSH houses 8 individuals. Transitional Housing is a one-year term lease. The service array includes: case coordination, case management, crisis intervention, mental health services, and medication support services for residents.

  • Permanent Housing Units – Nine studio and one – bedroom apartments housing one adult in each unit.
  • Transitional Housing – Housing for eight residents in shared housing unit. Residents have individual bedrooms and share common areas including living room, kitchen, baths, dining area.
  • Supportive Services – Supportive services will be provided for all residents provided by staff who have offices on the site. Sun Rose is also within walking distance of the Pajaro Wellness Center and Annex, operated by Interim, where additional peer operated supportive wellness services, as well as supported employment, and education services are available.
  • Accessibility – All units are handicapped accessible.

ADMISSION CRITERIA:

  • Financial Eligibility: Short-Doyle/Medi-Cal eligible or meet the standards for low-income status.
  • Tenants must meet HCD NPLH restrictions on income and assets as well as homelessness upon entry.
  • Referral through Coordinated Entry via the Homeless Coalition. All clients must have Interim or MCBH case coordinators with admission approval by Interim staff.
  • Adults with major psychiatric disabilities (including transition age youth age 18 and older) with serious mental illnesses, i.e. schizophrenia, schizoaffective disorder or bipolar disorders that substantially interfere on a long-term basis with their functional ability to carry out primary aspects of daily living in the community and are receiving psychiatry services through MCBH or Interim. (Exceptions to these criteria are only approved by MCBH and Interim Inc. Deputy Director or designees.)
  • Please note: Upon discharge from MCBHB services or Interim, rehabilitative mental health, case coordination, and case management services will be terminated. However, housing may not be terminated except as allowed under the lease. Upon discharge or termination from housing, clients are referred to MCBH case coordination services.
  • Housing eligibility is governed by funding sources regulatory agreements.
  • Admission eligibility determined by Interim Program Director or designee.

Referrals are only accepted through the Coordinated Entry via the Homeless Coalition. Following referral from Coordinated entry, MCBH Outpatient Clinics & Interim Inc. Programs – Interim Inc. or MCBH Case Coordinators may follow the process below.

Admission interviews are conducted Monday through Friday and set by Interim staff.

Once we receive your client’s referral information we will communicate with you to assure you know the referral was received and let you know when the interview is scheduled.
